Thank you for your desire to commit to the fellowship and ministry of Faith Life Church.

1 Corinthians 12:12-14 For as the body is one and has many members, but all the members of that one body, being many, are one body, so also is Christ. 13 For by one Spirit we were all baptized into one body—whether Jews or Greeks, whether slaves or free—and have all been made to drink into one Spirit. 14 For in fact the body is not one member but many.

I affirm my commitment to Jesus Christ as my Lord and Savior. I confess with my mouth the Lord Jesus and believe in my heart that God has raised Jesus from the dead. Jesus is Lord. I will continue in personal spiritual growth as a follower of Jesus Christ and will nurture my relationship with God by embracing the five principles and practices of the Journey of Faith.

WORSHIP: I commit to pursue intimacy with God through both private and public worship, Bible study and prayer to know and do the will of God.

INTEGRITY: I commit to live my life under the direction of the Holy Spirit and guided by the truth in God's Word, in order to live a life that gives daily evidence of the fruit of the Spirit, and Christ-like living.

FELLOWSHIP: I commit to participate in authentic community with other believers by being an active part of the church, to know and be known, love and be loved, and intentionally look after the interests of others.

MINISTRY: I commit to further the Kingdom through the mission of Faith Life Church by faithfully using my God-given spiritual gifts and talents in meaningful ministry

EVANGELISM: I commit to intentionally cultivate relationships with people who are spiritually unresolved and potentially far from God by investing, inviting and including them into the life and ministry of Faith Life Church.

I further commit to support the mission of Faith Life Church by: (Please check)

Trusting the Lord to be my provider, I commit to financially support the ministry of Faith Life Church. I understand the biblical precedent of percentage giving, and the accepted norm of a tithe. Acknowledging the biblical role of Pastors, Ministry Staff, Elders, and other spiritual leaders, I commit to be faithful in my followership of Christ under their leadership. Regularly praying for the pastors and other leaders, and ministry of Faith Life Church.

I recognize the voluntary nature of this covenant between God, Faith Life Church, and myself. My signature represents my desire and promise to share in the above privileges and responsibilities as a fellow brother/sister in Christ and at Faith Life Church. I understand that I may at any time void this commitment by notifying Faith Life Church. I give permission for this commitment to remain on file at Faith Life Church and to be accessible to the pastoral ministry team.
